Mission: We are a medical home which is an approach to providing total health care for our patients.  We are committed to providing accessible, patient centered, comprehensive care.  We strive to provide whole person care including preventative care, chronic condition management, and behavioral and mental health care.  In your medical home you, your family and your team of health care professionals will work together to develop your plan of care and promote self management.  Your medical home will help you access other medical professionals if needed and will connect you with community based resources.
